During the period from October 2020 to June 2021, mentees in the program actively participated in skills-building seminars, grounded in an antiracism framework, while simultaneously receiving support for their medical school application process. The present study sought to analyze the association of coronary angiography-derived microcirculatory resistance index (angio-IMR) post-primary percutaneous coronary intervention (PPCI) with the evolution of infarct characteristics over three months in patients who experienced ST-segment elevation myocardial infarction (STEMI).
A prospective study of patients with STEMI, who had undergone PPCI, encompassed the period from October 2019 to August 2021. Immediately subsequent to PPCI, Angio-IMR was calculated using a computational flow and pressure simulation. Cardiac magnetic resonance (CMR) imaging was performed, on average, 36 days and 3 months later. A total of 286 STEMI patients, comprising a mean age of 578 years, and exhibiting a male proportion of 843%, having received both angio-IMR and CMR examinations at baseline, formed the study group. In 84 patients (294% of the total), the angio-IMR level was significantly elevated, surpassing 40U. Patients with angio-IMR readings above 40U experienced a greater frequency and severity of MVO. Multivariate analyses demonstrated a significant association between angio-IMR values exceeding 40 units and infarct size, implying a three-fold higher risk of the final infarct size exceeding 25%. The adjusted odds ratio was 300 (95% CI 123-732, p=0.0016).
